Why downhill walking is so hard on your knees

On a descent, your job is not just to move forward, but to control your body’s fall downward. The steeper the slope and the longer your stride, the more speed your body gains between steps. Then that speed has to be absorbed—most often by the quadriceps and the knee joint.

That does not mean downhill walking is harmful in itself. For healthy legs, it can be good training. The problem starts when you walk as if you were on flat ground: you place your foot far ahead, lean back, and turn every step into a small braking action.

In a laboratory descent at 25°, walking with poles reduced peak and average measures of ground reaction force, knee moment, and tibiofemoral forces by about 12–25%. The authors linked the effect not only to pressure on the poles, but also to a slightly more forward body lean, which reduces the knee moment arm.

The main principle: don’t brake with your knees

Imagine that the knee is not a handbrake, but part of a shock absorber. A good descent does not feel like fighting the mountain; it feels like a series of short, quiet, controlled steps. Your goal is to keep your center of mass closer to the foot that is already on the ground, instead of catching your body far out in front.

If your thighs burn after a descent, that’s normal. If every step sends sharp pain into your knee, that is not heroic technique—it is a signal to reduce the load.

  • Look 2–3 steps ahead, not right under your nose: this helps you choose foot placement in advance.
  • Do not take a wide lunge downhill, even if you want to finish the descent faster.
  • Keep your knees soft: do not lock them fully straight at the moment of contact.
  • Place your foot quietly: the sound of impact is often more honest than your sense of speed.
  • If the slope is steep, go in a zigzag—this reduces the actual angle of descent.

Stride: shorter, quicker, quieter

The fastest technique fix is to shorten your stride by 10–20%. You do not need to deliberately shuffle, but your foot should not fly far ahead of your body. When your stride is shorter, you “plant” your heel into the slope less and transfer weight through the foot more easily.

  1. Find your normal stride on a gentle descent.
  2. Shorten it so your foot lands almost under your pelvis, not far in front.
  3. Add a little more cadence: let speed come from rhythm, not from a long fall downhill.
  4. Keep your knees soft, like springs, but do not squat deeply on every step.
  5. Check the sound: quiet steps are almost always kinder to the knees.

In a study of level walking, reducing step length while maintaining speed immediately lowered some measures of knee joint load. This was not a direct study of mountain descents, but the principle transfers well into practice: a shorter stride reduces leverage and helps you avoid braking with the joint.

Body and pelvis: a light lean, not falling backward

The most common downhill mistake is leaning back. It feels safer, but it often increases braking: the foot moves forward, the knee catches the body, and the step becomes stiff. It is better to keep your torso tall and slightly directed down the slope—as if you want to follow your feet, not run away from them.

If, on a descent, you feel your heels “slamming” into the ground, try bringing your chest slightly forward and shortening your stride. Do not bend from your lower back: the movement comes from the whole body, softly and calmly.

Arms and poles: move some of the work upward

  • On a normal descent, place the pole slightly ahead and to the side of your foot, but not too far from your line of travel.
  • On a steep section, you can use a double plant: both poles lower down the slope, then step.
  • Do not twist your shoulders or place the poles too wide—you lose support that way.
  • Press down on the handle rather than pulling the pole back like an oar.
  • In cold weather and at altitude, do not make the poles excessively long: your hands should not stay above your elbows all the time.

15 experienced male hikers descended with and without poles, carrying different backpack loads: no load, 15%, and 30% of body mass. Poles reduced sagittal moments at the ankle, knee, and hip, and also reduced peak power absorption at the ankle and knee. In other words, they helped distribute the work, especially when there was a backpack.

Trekking poles reduce exercise-induced muscle injury during mountain walking
In a field study, 37 participants climbed and descended Mount Snowdon. The group using poles better maintained maximal voluntary muscle contraction, reported less soreness after 24 and 48 hours, and had lower creatine kinase after 24 hours. This is an important argument for poles not only for the knees, but also for recovery after a long route.

Pace: slow is not always soft

Going downhill too fast obviously raises the risk: you react later, brake harder, and make more foot-placement mistakes. But going excessively slowly can also be demanding if you hang on one leg and hold your body with your quadriceps for too long.

Look for a steady, controlled rhythm. A good marker: you can speak in short phrases, your feet land quietly, and pain does not increase as you descend. If the knee starts to “ache more and more,” that is not a reason to endure until the end—reduce the angle, switch to a zigzag, take the railing or poles, and pause.

The pain rule

For a walk, mild discomfort is acceptable if it does not increase and goes away after the load. If pain is above 2–3 out of 10, changes your gait, or lasts into the next day, reduce the volume and contact a doctor or physiotherapist.

Stairs and urban descents

Urban descents are not only hills. Stairs, underpasses, and steep paths down to the waterfront also require braking. In a study with instrumented knee implants, the average peak resultant force was higher when descending stairs than when climbing stairs and during normal walking. So technique on steps is not a small detail.

  • Place your whole foot on the step if the width allows, rather than hanging on your toes.
  • Use the railing without embarrassment: it is the same kind of extra support as a pole.
  • When tired, descend with a step-to pattern: first one foot, then the other onto the same step.
  • Do not skip steps if your knees are already sensitive.
  • Keep your body over your foot, rather than leaning back toward the upper step.

In 5 people with instrumented knee implants, loads were measured during different everyday movements. The average peak resultant force was highest when descending stairs—346% of body weight, higher than when climbing stairs and walking on level ground. This does not mean stairs are forbidden, but it explains why technique and railings matter so much.

Route, shoes, and backpack

  • Tighten your laces so your foot does not slide forward and your toes do not hit the toe box.
  • On slippery surfaces, shorten your stride even more and place your foot flatter.
  • If you carry water, distribute the weight closer to the center of your back.
  • On a long route, plan stops before your thighs start shaking.
  • After getting new shoes, test the descent on a short hill first, not on a big route.

A mini-workout before the descent

If your knees hurt specifically going down, what is often missing is not “courage,” but the strength to control movement. Descending requires eccentric work: the muscles contract while lengthening.   1. Step-down from a low step: slowly lower the free heel toward the floor, 2–3 sets of 6–8 reps per leg.
  2. Slow squats to a comfortable depth: 3 seconds down, calmly up.
  3. Calf raises: your calves help control the foot on an uneven trail.
  4. Glute bridge: the pelvis and hip should help the knee, not dump everything onto it.
  5. Practice descending on a small slope: 5 minutes of short, quiet steps instead of one big mountain once a month.
Progress like a route, not like a record

Add difficulty one parameter at a time: first descent length, then steepness, then backpack. If you change everything at once, you will not understand exactly what overloaded the knee.

When pain is a reason to stop

A descent can bring fatigue in the front of the thigh, gentle warmth in the muscles, and a desire to slow down. But it should not bring sharp pain, a feeling of instability, or swelling. In those cases, technique is no longer the main question—you need to understand the cause.

  • Stop if the knee “gives way” or you feel it locking.
  • Do not continue descending through sharp pain under the kneecap or along the inner or outer joint line.
  • Pay attention to swelling on the day of the load or the next morning.
  • After an injury, surgery, or joint replacement, return to descents only according to a specialist’s plan.
  • If pain repeats on every staircase, start with short routes and a strength base, not with mountains.

Do I always need to use poles on descents?

Not always. On a short urban hill, technique and pace may be enough. But on a long descent, with a backpack, on a slippery trail, or with sensitive knees, poles often help distribute the load and add stability.

Is it better to land on the heel or the whole foot?

Do not fixate on one contact point. On a gentle descent, a soft roll from the heel can work; on dirt, a flatter foot placement is often more comfortable. The main thing is not to plant the heel far ahead of the body and not to strike the ground with the foot.

Why do my knees hurt going down, but almost not going up?

Going up, you mostly create force; going down, you absorb the body’s movement. The quadriceps work eccentrically, and the knee gets more of the braking job. That is why descending is often harder specifically for the front of the knee.

Can I train downhill technique on stairs?

Yes, but dose it carefully. Start with low steps, a railing, and slow control. If your knee aches the next day after stairs, reduce the volume and add pain-free strength exercises.

What should I do if my knee already hurts halfway through the route?

Immediately shorten your stride, slow down, switch to a zigzag, and use poles or a railing. Take short pauses. If the pain is sharp, the knee feels unstable, or swelling appears, it is better to finish the route by a safe way and not push through the descent by sheer tolerance.
